As with any successful relationship, a dentist-patient relationship depends on teamwork. If you do your part in preparation for your first visit, we will do ours to make sure you get the dental care you need and desire.
Patient Registration
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns, and we have taken every precaution to protect it.
Preparing for Your First Visit
In order to prevent dental disease, any active disease must be evaluated and eliminated. To do this successfully, we must have a complete picture of your present oral conditions, as well as insight into your dental and medical history. We will make a thorough evaluation of existing and potential problems, taking the necessary x-rays, and examining the oral tissues for decay, oral cancer, and gum or bone disease.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable
- A list of medications you are presently taking
- Completed dental insurance forms.
X-Rays
If your dentist or physician has taken x-rays, we can request them using the information you provide in your pre registration form. It also helps if you request that they forward them to our office. If additional x-rays are necessary for diagnosis we are able to take those while you are here for your appointment.
What to Expect on Your First Visit
During your first visit, Dr. Skowronski will take time to get to know you. He will ask about your oral health history and your previous experiences with dentistry. He will also ask about your oral health goals and any concerns you might have about your current oral health.
Dr. Skowronski will perform a comprehensive exam. He will consider what he sees in the context of your medical history and goals. Then he will let you know if you have any imminent oral healthcare needs or if we can simply monitor your oral health for now.
